Registration of New Business Name
1. NAME
The name of your first preference will be registered if it is considered to be available by the Corporate Affairs Office. Should the name be unavailable, and if you would be prepared to use a different business name, please indicate your second preference.
If the name shown as your first preference is your only preference - and if it is unavailable - please advise if you wish to conduct a search of the blocking business name/company name to be made.
2. REGISTERED OFFICE
This may be the same as the address of the business - or it may be the address of your accountants or solicitors. The address must be in Victoria.
3. BUSINESS ADDRESS(ES)
There may be more than one address you use for your business. If so, please provide all addresses on a separate page.
4. NATURE OF BUSINESS
A specific but brief description is required.
5. DATE BUSINESS COMMENCED
This must be a date not more than two months later than the registration date.
6. OWNER(S) OF BUSINESS
Please provide the full name and residential address for each owner. If the owner is a company, the address should be the registered office of the company. Please advise the company’s ACN in the section headed date of birth
Each owner is required to sign the form. If the owner is a company, a director, secretary or manager of that company must sign the form.
7. OTHER
A business name is registered for three years. A renewal form will be sent to the registered office of the business name shortly before the expiry date.
